When you’re driving up a steep gradient or sitting at threshold, your body naturally shunts blood away from your digestive tract to feed your working legs. If you are taking a GLP-1 medication like Ozempic, that delicate gastrointestinal system faces an added challenge. Let’s take a closer look.

The core mechanism of GLP-1 drugs is delayed gastric emptying. This means that food stays in your stomach significantly longer than it normally would. While this works wonders for curbing appetite and keeping you full, it can easily turn into nausea, bloating, acid reflux or sloshing during high-intensity efforts.

Managing stomach distress on a GLP-1 isn’t about giving up interval sessions or fast group rides. It comes down to understanding how your digestive system reacts under stress and adapting your fuelling tactics accordingly.

The anatomy of exercise-induced GI distress

Under normal conditions, simple carbs pass quickly through the stomach and absorb rapidly in the small intestine, delivering quick energy to your muscles.

On a GLP-1 medication, that conveyor belt slows down. If you consume dense bars, fats or overly concentrated sports drinks before or during a ride, that food sits stagnant in your stomach. Add the physical bounce of pedalling, elevated heart rates, and reduced abdominal blood flow, and you have a recipe for severe GI distress.

And on top of that, these medications can hide your body’s thirst signals just as much as your hunger cues. Many riders don’t realise they are under-hydrated until a sudden spike in heart rate or dizziness hits mid-ride. Because proper hydration is required for smooth digestion, riding in a dehydrated state makes delayed gastric emptying even more uncomfortable.

To be able to handle hard training sessions without feeling sick, you need to adjust what, when, and how you take in fuel.

Reduce your pre-ride meal timing

The classic “eat two hours before a big ride” rule often needs to be stretched when on a GLP-1. Give your stomach extra time to process your last big meal before a ride. Aim for 2,5 to 3 hours before a hard session. If you need a top-off right before rolling out, stick to fruit or energy gels to make sure there is no fat, fibre or protein to slow down digestion.

Avoid highly concentrated fuel shots

Gulping down a thick energy gel without enough water creates a high osmolarity in your stomach, drawing fluid into the GI tract and causing cramping. Dilute your carbohydrates by using liquid drink mixes that combine hydration with simple sugars, sipping small amounts consistently rather than taking heavy doses all at once.

Stay ahead on electrolytes and fluid

Since thirst cues can be muted, hydration must become a disciplined routine rather than a response to feeling dry. Drink small amounts of an electrolyte-enhanced mix every 10 to 15 minutes. Sodium helps your body absorb fluids more efficiently in the gut, reducing that uncomfortable “water balloon” sloshing feeling in your stomach.

Scale your fuelling to your intensity

On low-intensity, conversational-paced rides, your digestive system can handle a bit more. But on days with hard intervals or fast racing efforts, dial back the total grams of carbohydrate per hour. You will have to experiment with how many grams of carbs per hour you have to reduce compared to normal.

The big picture

Slower digestion that comes with GLP-1 drugs shouldn’t keep you from riding hard. By giving your digestive system more time to process food, opting for simple, diluted carbs, and staying disciplined with electrolytes and hydration, you can keep stomach trouble to a minimum. These tactics are nothing new, but they become significantly more important on your GLP-1 journey.

Pushing hard on a bike is already tough on the stomach.
